Medical record documentation is an essential component in the care of the dialysis patient.  While many dialysis staff members will admit that medical documentation is important, many also complain about the lack of time to document. 

We often hear the question "Well, what do you want me to do: take care of the patient or take care of the paper work (document)?"   While the hectic pace of a dialysis unit can have time constraints, we can not choose one task (documentation) over the other. They both must be done. 

Why? Documentation serves some very useful purposes: 

  1. As a form of communication by and between the team members caring for the patient.  This includes the direct and non-direct caregivers, and the physicians. 
  2. As a permanent record of the events surrounding the care of the patient and the patient's response to the care. 
  3. For reimbursement purposes. 
  4. To CYOA (Cover Your Own Anatomy).

The medical record is a legal document and should be treated as such.
 
In the event of a lawsuit, lawyers, judges, jurors and medical experts hired by the plaintiff's attorney will review the records. The purpose of their review is to prove the allegations of medical negligence.
